Description: This session will provide an overview of one of the Ohio Health Information Exchange (HIE) systems, CliniSync, and describe how this tool can be used by behavioral health organizations to support care coordination across health systems and providers. Participants will learn how HIE can be utilized to exchange clinical data and improve patient outcomes.

Objectives:

  • Summarize the CliniSync HIE connectivity and reach throughout Ohio.
  • Compare the breadth of clinical data exchange occurring within Ohio with other states/regions.
  • Demonstrate how to utilize the CliniSync HIE tool suite to access clinical data.
  • Summarize how the CliniSync HIE toolset supports Care Coordination and Population Management.
